Influence of residual NFAT-regulated gene expression on ciclosporin A-induced gingival hyperplasia
Gingival proliferation under CsA therapy develops in 30-40% of patients. Although the effect of CsA on lymphocytes is largely known, the CsA-related pathogenesis of gingival overgrowth is still unclear. Since not all patients treated with cyclosporine develop gingival overgrowth, it is now assumed that the pathogenesis of gingival overgrowth is multifactorial. The following factors are included in this multifactorial model:
- Age and gender
- genetic factors (e.g. cytochrome P450, HLA antigens)
- pharmacokinetic variables and
- the use of other medications (e.g. calcium antagonists)
In addition to these factors, it is now considered certain that there is a connection between the prevalence and severity of drug-induced gingival proliferation and plaque-induced gingival inflammation (gingivitis).
In a joint project with the dental clinic, transplanted patients are currently being screened for gingival hyperplasia. At the same time, clinical influencing factors and residual NFAT-regulated gene expression are being recorded as an indication of the individual efficacy of Ciclosporin A.
